INF SelectCare Insurance Policy (25/50/75/100K) Claims

Please use the online insurance enrollment form for reenrollment of an existing plan. You may reenroll for up to 12 months of continuous coverage. Complete the form in its entirety. The last step will produce Summary of Benefits and Insurance ID Cards. If you have problems with online enrollment forms or need assistance in completing the forms, please contact our office for assistance at 408-540-3600.
Please use the online links to add existing visitors, update a membership or to change your address. If your membership has expired and you want to purchase health insurance for a visitor, use the Online Health Insurance Enrollment form and check "Please Renew my India Network membership ($10)" when prompted in Step 1 of the online application form. Cancellation is applicable only if occurs before start date of coverage, and it requires $25 processing fee. Please fill out the cancellation form, and fax your completed form to 408-520-4967.
Cancellation or refund are not offered on or after effective date of insurance.
Note: Cancellation applies to the whole policy (regardless of number of people insured).
First Health Network
India Network Foundation’s Standard and Premier Insurance Plans allow insureds to choose a doctor or hospital of their choice. To help our insureds enrolled in the INF Elite Coverage find a provider participating in a network, India Network provides access to the First Health PPO Network. contact First Health at 888-293-9229.
Consult a Physician for AXIS policy holders
The Teladoc program is to help visiting parents and others who need to consult a physician during their stay in the United States or Canada for questions on medications, or pre-existing ailments such as Diabetic, Blood Pressure, Pain management, and for minor non-emergency physician consultations. This service is provided for the benefit of the BasicCare Plan Participants. Program is available 24 x7 and can be reached Teladoc office by 1-800-TelAdoc.
Consult a Nurse for CHUBB Policy holders with numbers GLM
The purpose of this program is to help visiting parents and others who need to consult a medical professional such as a Registered Nurse during their stay in the United States or Canada for questions on medications, or pre-existing ailments such as Diabetic, Blood Pressure, Pain management etc. This service is provided for the benefit of the India Network Health Insurance Participants. Program is available 24 x7 and can be reached a RN by calling 1-855-428-3425.
